Contracted Care Management Unit for Milwaukee County Department of FamilyCare.  We are responsible for monitoring the long term care needs individuals with developmental disabilities and older adults who receive support services in Milwaukee County. We ensure that individuals have the appropriate levels of support services to live as safely and independently as possible. (Program provided in Milwaukee County only.)
